Depending on the sponsorship level chosen, your information may be used for our website, local advertising campaigns, social media sites, a recognition plaque or for our end of year volunteer appreciation event. After completing this form, you will be able to pay the sponsorship fee online or you may send a check (made payable to AYSO Region 678) to the following address:
