Have you been injured?

All registered players and volunteers are covered for accidental bodily injury while participating in AYSO sanctioned events. Please know however that it is not 100% coverage and all claims must be filed within 90 days of the date of injury! If you have any questions, please contact our insurance and safety coordinator at safety@ayso678.org .

Where do I get the Insurance Forms?

Please go to www.soccer.org and click on AYSO Downloads, hold the cursor on FORMS & DOCUMENTS, and then click on INSURANCE FORMS. Download the SAI claim and instructions. If you have any questions, please contact our insurance and safety coordinator at safety@ayso678.org .

Below is a simplified summary of the insurance coverage:

• All claims must be filed within 90 days of the date of injury
• You are responsible for a $200 Deductible
• The AYSO coverage is a Full Excess policy which pays at Usual & Customary levels after your Primary insurance has paid
• There is a $1000 limit for Physical Therapy/Chiropractic visits
